Faith Andrews, MD
Faith A. Andrews M.D. received her Bachelor of Science degree from St. Mary’s University in San Antonio, Texas; with honors. She earned her Medical Degree from Howard University College of Medicine, in Washington D.C. in 1996, and went on to complete her internship and residency at Mercer University (Medical Center of Central Georgia) in 1999. After graduating she worked at an indigent clinic in Griffin, Georgia.
She has been practicing in the Southern Crescent area since 2000. She opened Southern Crescent Family Practice in Forest Park, GA in 2004 to fulfill a lifelong calling. One of Dr Andrews’s gifts is the ability to educate patients about their medical conditions.
It is often said that Dr. Andrews “does not talk at you, she talks with you.” Southern Crescent Family Practice is a family focused office. You will never be treated like “just another patient.” We pride ourselves on providing competent, compassionate and quality medical care.
In her spare time Dr. Andrews enjoys swimming, cooking, reading, concerts and spending time with her husband and four beautiful children.
Melinda Chavis
Melinda Chavis is a Certified Nurse Practitioner.
She is a 1991 Graduate of The University of South Carolina where she obtained her undergraduate degree in Nursing.
She obtained her Masters' Degree in Nursing in 2012 from Georgia State University. Her interests include: gardening, Dachshund Rescue, cooking and spending time with family and friends.
Melinda enjoys educating her patients and empowering them to take an active role in their healthcare.
Sandy Mcintosh-Takori
Graduated from Sullivan County College of Nursing in N.Y with an A.S degree as an RN in 2004 and worked for Catskill Regional Medical Center in Harris, N.Y. from 2005-2006 on a medical/surgical unit.
Sandy then did travel nursing in N.Y after. I moved to GA in 2006 and worked for Wellstar Health systems,Cobb Hospital as an RN from 2006-2020, where she worked in Orthopedics and later the pre op clinic and pre and post op areas. Received B.S in Nursing from Chamberlain College of Nursing in 2010 with President's honors.
Received MSN as Family Nurse Practitioner in 2016 from South University. I worked at Temple Medical Clinic in Villa Rica then Temple, Ga as an FNP from 2018-2019,then for Abella Palliative Care in Calhoun,Ga from 2019-2022. Matrix Medical Network from 2023.
Gennell White
Gennell White, FNP-BC, is a dedicated Family Nurse Practitioner with over 18 years of experience in nursing. She provides comprehensive, patient-centered care across the lifespan, with a strong focus on preventive health, chronic disease management, and health education.
Gennell is passionate about building trusting relationships with her patients and believes in empowering individuals to take an active role in their health. She is known for her compassionate approach, attention to detail, and commitment to delivering safe, high-quality care.
Her clinical interests include managing conditions such as hypertension, diabetes, and other chronic illnesses, as well as promoting overall wellness through routine screenings and lifestyle guidance.
She is passionate about providing personalized, compassionate care and takes pride in making patients feel comfortable, heard, and respected.
Evelyn Adio
Evelyn Adio, RN, MSN-FNP, AACNP. Evelyn graduated with honors from the Chamberlain College of Nursing's Master of Science Program, specializing as a Family Nurse Practitioner.
She obtained a Bachelor of Science in Nursing from Jacksonville University School of Nursing in Florida and an Associate of Science Degree in Nursing from Century College/Inver Hills Community College in Minnesota. She started her career as a cardiac nurse at Allina Healthcare in St. Paul, Minnesota, before relocating to Georgia's Emory Healthcare System, where she worked with diverse patient populations from 2006.
She has been a per diem staff member at Southern Crescent Family Practice since 2019. Evelyn is a Board-Certified Family Nurse Practitioner and a member of the Honor Society of Nursing, Sigma Theta Tau International, and the American Academy of Nurse Practitioners. She emphasizes patient-centered care, focusing on education and preventive medicine.
